How did you hear about Accomplished Artist Studio?

I understand that this application form is the first step in being considered a good candidate for The Academy program and that I am not yet enrolled.

I understand The Academy is a weekly 2 1/2 hour class.

I understand tuition is $240 per month, and I will be automatically charged on the 25th of each month.

I understand that this is an ongoing program with no contract and no end date.

I understand that there are no discounts or make ups for missed classes, but the teacher will do their best to help the student learn what they have missed.

I understand that if I want to remove myself from The Academy program I must do it before the 20th of the month so I am not charged tuition. No refund for tuition already paid unless authorized.

I understand that there is an enrollment fee of $40 when enrolling and each September thereafter. This covers the cost of a new studio t-shirt, contests and events.
